SpeakersTom Weber, Director of Care Management Services at SAGE.  

Tom is the administrator for SAGE’s social services, case management, caregiving support, friendly visiting, veterans and SAGEPositive (sexual health) programs.  Tom has worked at SAGE for over 12 years as Director of Community Services, coordinated SAGE’s NYC Gay Pride efforts, and was involved in the launch and early years of SAGEWorks, SAGE’s employment assistance program serving LGBT adults age 40 and over. 

Prior to coming to SAGE, Tom worked for almost 15 years at Gay Men’s Health Crisis (GMHC). In his last role there, he was Director of the Buddy Program, one of the agency’s signature programs, pairing volunteers with home-bound people living with AIDS, for companionship and emotional support.
